Community Conduct Form

Everything we do and believe at Generation Exchange is based on a community of respect and trust. It is of utmost consequence that we sustain those values at all times within the Generation Exchange community. As such, we respectfully ask each participant as well as our partners to commit to these principles and code of conduct :

Integrity is vital. This means respecting your partner regarding security issues such as passwords and log-ins as well as other confidential information. In other words, please do not put your Mentor or Mentee in a compromised position by either offering or requesting such confidential information.

Inclusion is a given. Generation Exchange is driven by the intention of bringing empowerment and connection to as many as wish it and seek it (...that we can accommodate !) At times we must limit participants based on space and resources, but we never do so on the basis of ability, financial means, race, color, national origin, sex or religion.

Consideration is a necessary element of respect and trust. We strive and succeed in maintaining a space that is encouraging and reassuring to all participants. We retain the sole right of determining when inconsiderate behavior disturbs the space or participants in Generation Exchange workshops. We will do our best to express this concern to the participant who is the source of the disruption, and we will seek to resolve it, but if not resolved we necessarily will require the participant to leave the workshop at that time.

CCOP Liability Waiver

Notice to Participants of Risk and Waiver of Responsibility 

I exercise my own free and voluntary choice to participate in the designated activity, including use of facilities and equipment provided by the Catholic Community of Pleasanton (CCOP) and Generation Exchange. I understand and assume all associated risks of the designated activity.  I agree to assume all risk of personal injury or loss, bodily injury (including death), damage to or loss of, or destruction of any personal property resulting from or arising out of participation in the designated activity. I also release, waive, indemnify, hold harmless, and discharge the Catholic Community of Pleasanton (CCOP) and Generation Exchange from all claims, damages, and injuries arising out of my activities, including my use of equipment and facilities provided by the Catholic Community of Pleasanton (CCOP) and Generation Exchange.

I agree to take full responsibility for any and all changes that occur to my device(s) as a result of my participation in Generation Exchange. Generation Exchange and its volunteers are in no way responsible for any loss of data, changes in settings, or dysfunctional devices.

The Catholic Community of Pleasanton (CCOP)and Generation Exchange do not provide health insurance for individuals participating in activities made available or sponsored by the University of Denver or Generation Exchange. As such, you or your personal health insurance will be responsible for payment of medical services and care for any injuries sustained during the designated activity.

For participants under 18 years of age, the participant’s parent or legal guardian accepts these terms, consents to the minor’s participation, and agrees to the assumption of risk and release of liability provisions on behalf of the minor, to the fullest extent permitted by California law.

 Photo Release Form

I do hereby authorize Generation Exchange and those acting pursuant to its authority to:

a. Record my participation and appearance on Videotape, 

Audiotape, Film, Photograph or any other medium

b. Use my name, likeness and voice in connection with these recordings

c. Exhibit or distribute such recording in whole or in part without restrictions or limitation for any educational or promotional purpose which Generation Exchange and those acting pursuant to its authority deem appropriate.
