Community Conduct Form

Everything we do and believe at Generation Exchange is based on a community of respect and trust. It is of utmost consequence that we sustain those values at all times within the Generation Exchange community. As such, we respectfully ask each participant as well as our partners to commit to these principles and code of conduct :

Integrity is vital. This means respecting your partner regarding security issues such as passwords and log-ins as well as other confidential information. In other words, please do not put your Mentor or Mentee in a compromised position by either offering or requesting such confidential information.

Inclusion is a given. Generation Exchange is driven by the intention of bringing empowerment and connection to as many as wish it and seek it (...that we can accommodate !) At times we must limit participants based on space and resources, but we never do so on the basis of ability, financial means, race, color, national origin, sex or religion.

Consideration is a necessary element of respect and trust. We strive and succeed in maintaining a space that is encouraging and reassuring to all participants. We retain the sole right of determining when inconsiderate behavior disturbs the space or participants in Generation Exchange workshops. We will do our best to express this concern to the participant who is the source of the disruption, and we will seek to resolve it, but if not resolved we necessarily will require the participant to leave the workshop at that time.

 Photo Release Form

I do hereby authorize Generation Exchange and those acting pursuant to its authority to:

a. Record my participation and appearance on Videotape, 

Audiotape, Film, Photograph or any other medium

b. Use my name, likeness and voice in connection with these recordings

c. Exhibit or distribute such recording in whole or in part without restrictions or limitation for any educational or promotional purpose which Generation Exchange and those acting pursuant to its authority deem appropriate.

Notice to Participants of Risk and Waiver of Responsibility 

I exercise my own free and voluntary choice to participate in the Generation Exchange activities, including use of facilities and equipment provided by the University of Colorado. I understand and assume all associated risks of the designated activity. 

I agree to take full responsibility for any and all changes that occur to my device(s) as a result of my participation in Generation Exchange. Generation Exchange and its mentors are in no way responsible for any loss of data, changes in settings, or dysfunctional devices.

I agree to assume all risk of personal injury or loss, bodily injury (including death), damage to or loss of, or destruction of any personal property resulting from or arising out of participation in the designated activity. I also release, waive, indemnify, hold harmless, and discharge the University of Colorado and Generation Exchange from all claims, damages, and injuries arising out of my activities, including my use of equipment and facilities provided by the University of Colorado and Generation Exchange. 

The University of Colorado and Generation Exchange do not provide health insurance for individuals participating in activities made available or sponsored by the University of Colorado and Generation Exchange. As such, you or your personal health insurance will be responsible for payment of medical services and care for any injuries sustained during the designated activity. 

I hereby certify that I have read and understand the provisions above. For participants under 18 years of age, the parent or guardian accepts the above terms and grants permissions for the student’s participation on behalf of said minor, as permitted by C.R.S. § 13-22-107. 

 CSU Liability Waiver

Notice to Participants of Risk and Waiver of Responsibility 

I, the undersigned participant, exercising my own free choice to participate voluntarily in the activities described above, promising to take due care during such participation, hereby acknowledge that I have been informed of the nature of activities and that I am aware of the hazards and risks which may be associated with my participation in the above-named activities, including the risks of bodily injury or death, or damage to property which may occur from known or unknown causes.

I understand, accept, and assume all such hazards and risks of injury or damage, and waive all claims against the State of Colorado, The Board of Governors of the Colorado State University System, Colorado State University, Generation Exchange, and other persons as set forth above. I understand that I am solely responsible for any costs arising out of any bodily injury or property damage that I may sustain through my participation in normal or unusual acts associated with the above-named activities, regardless of whose fault may be the cause of my injuries or damages, EVEN IF CAUSED BY CARELESSNESS OR NEGLIGENCE, so long as the conduct which caused the injuries or damages was not grossly negligent, or willful and wanton.

I agree to take full responsibility for any and all changes that occur to my device(s) as a result of my participation in Generation Exchange. Generation Exchange and its mentors are in no way responsible for any loss of data, changes in settings, or dysfunctional devices.

Further, I hereby indemnify and hold harmless Generation Exchange, The Board of Governors of the Colorado State University System and Colorado State University, and their members, officers, agents, employees, and any other persons or entities acting on their behalf, and the successors and assigns for any and all of the aforementioned persons and entities, against any and all claims, demands, and causes of action whatsoever, whether presently known or unknown, of any person who suffers any injury, disability, death or other harm, to person or property or both, as a result of my participation in and/or presence at the above listed activities.

I have had sufficient time to review and seek explanation of the provisions contained above, have carefully read them, understand them fully, and agree to be bound by them. After careful deliberation, I voluntarily give my consent and agree to this Release From Responsibility, Assumption of Risk, and Waiver.
